The Assessment Nurse is a critical role within MetroPlusHealth, responsible for conducting comprehensive assessments to determine the level of care needed for applicants and members. This position requires a strong clinical background, excellent communication skills, and the ability to work effectively with diverse populations.
Key Responsibilities- Conduct UAS assessments to determine the level of care needed for applicants and members
- Meet with applicants, family members, and caregivers to discuss assessment results and determine eligibility for the MLTC program
- Complete the UAS assessment tool and make determinations regarding program enrollment
- Conduct reassessments of members as scheduled or upon request from the Care Manager
- Participate in quality management activities related to MLTC and provide information to management teams
- Bachelor's Degree in Nursing
- Minimum of 2-3 years of clinical experience in CHHA, Lombardi program, and MLTC
- Registered Nurse licensure in New York State
- UAS Certified RN required
