Job Summary:

As a UAS Assessment Nurse with MJHS, you will play a vital role in conducting home visits to assess the needs of our members. With your keen professional judgment and strong persuasive power, you will determine eligibility and establish individual plans of care that enable our members to remain safely at home despite long-term care needs.

Responsibilities:

• Conduct functional, psychosocial, and environmental safety assessments in the home setting
• Determine eligibility for services and establish individual plans of care
• Collaborate with interdisciplinary teams to provide comprehensive care
• Communicate effectively with culturally diverse populations, including caregivers and other providers

Requirements:

• Current NYS RN license and registration
• Associates degree required; Bachelor of Nursing degree preferred
• Bilingual BSN new graduates encouraged to apply
• One year of experience in Homecare, acute care, or community health nursing preferred
• Working knowledge of Medicare, Medicaid, and NYS DOH standards and regulations
• Bilingual English/Spanish, Mandarin, Cantonese, Korean, Creole, Bengali, Hindi, and Arabic desirable
