Responsibilities:
As a UAS Assessment Nurse, you will conduct home visits to assess members' needs, determine eligibility, and establish individual plans of care. You will work with an interdisciplinary team to provide support and ensure high-quality care.
We have multiple openings in various regions, including the Bronx, Queens, Nassau, Manhattan, Westchester, and upstate counties. Our Assessment Nurses receive a higher reimbursement rate for conducting extra visits.
Qualifications:
- Current NYS RN license and registration required
- Associates degree required; Bachelor of Nursing degree preferred
- Bilingual BSN new graduates encouraged to apply
- One year of experience in Homecare, acute care, or community health nursing preferred
- Working knowledge of Medicare, Medicaid, and NYS DOH standards and regulations
- Bilingual English/Spanish, Mandarin, Cantonese, Korean, Creole, Bengali, Hindi, and Arabic desirable
