About the Role

We are seeking a highly skilled and compassionate Human Services Specialist to join our team at Dane County, WI. As an Information and Assistance Specialist, you will play a critical role in connecting older adults and adults with disabilities to vital programs and services.

Key Responsibilities
  • Provide information and assistance to customers, connecting them to public and private community resources.
  • Assess customer needs and provide options for both public and private community resources.
  • Administer the state's Long Term Care Functional Screen and assist customers with applying for public benefits.
  • Determine functional eligibility and assist in determining financial eligibility for publicly funded long-term care programs.
Requirements
  • Bachelor's Degree in a Human Services related field (Social Work, Sociology, Psychology, Nursing, etc.).
  • One (1) year of experience in an aging and/or disability related professional position.
  • Valid driver's license and access to reliable personal transportation.
  • Ability to work collaboratively with colleagues and collaborating partners.
Preferred Qualifications
  • Bilingual or multilingual skills.
  • Experience working with older adults and adults with disabilities.
  • Knowledge of community resources and referral procedures.
