About the Role:
  • Provide comprehensive assessments, develop and implement plans of care, and coordinate patient visits.

  • Document patient information accurately and efficiently in our electronic health record system.
Requirements:
  • ADN or BSN degree required; current, unrestricted RN license in the state of employment.
  • One year of nursing experience preferred; ability to develop and maintain knowledge of hospice and end-of-life care.
  • Excellent communication and interpersonal skills; ability to work effectively in a team environment.
  • Basic Microsoft Office skills; intermediate electronic health records skills (Epic preferred).
