About the Role:- Provide nursing care to patients and families in a hospice setting, including case management, patient visits, and admission visits.
- Conduct comprehensive assessments, develop and update plans of care, and provide interventions to meet patient needs.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Document patient information accurately and efficiently in our electronic health record system.
- Collaborate with our interdisciplinary team to ensure patient-centered care and achieve optimal outcomes.
- ADN or BSN degree required; current, unrestricted RN license in the state of employment.
- One year of nursing experience preferred.
- Valid driver's license, auto insurance, and access to a reliable vehicle.
- Ability to develop and maintain knowledge of hospice and end-of-life care processes.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
