About the Job:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at CenterWell Home Health.

As an LPN, you will play a vital role in providing high-quality patient care in the comfort of patients' homes. Our team works collaboratively to ensure that each patient receives personalized attention and support to achieve their unique goals and outcomes.

About the Job Responsibilities:

  • Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state laws, regulations, and CenterWell Home Health policies.
  • Assess patient needs, develop individualized care plans, and implement interventions to promote patient health, safety, and well-being.
  • Communicate effectively with patients, families, caregivers, and healthcare teams to ensure seamless coordination of care and optimal patient outcomes.
  • Maintain accurate and comprehensive documentation of patient interactions, assessments, and care plans.
  • Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.

About the Ideal Candidate:

  • Graduate of an accredited Licensed Practical Nursing program or vocational school.
  • Current licensure as an LPN in the practicing state.
  • Valid driver's license, reliable transportation, and automobile insurance.
  • Two years of experience as an LPN in a clinical setting, preferably in home health or hospice care.
