Nurse Case Manager

Professional Case Management is seeking a skilled Registered Nurse to join our team as a Case Manager. As a Case Manager, you will be responsible for conducting in-home nursing visits for our clients, who are former Nuclear Weapon's Workers with chronic/terminal illness due to exposure to toxic substances while working for the Government.
Key Responsibilities:- Conduct in-home nursing visits for clients, including physical assessments, medication management, and home safety evaluations.
- Provide case management services to assigned clients, including coordinating care with healthcare providers and family members.
- Supervise a team of RNs, LPNs, and caregivers to ensure high-quality care is delivered to clients.
- Collaborate with regional directors and other personnel to identify and correct issues and improve services.
- Graduate of a state-approved school of professional registered nursing.
- BSN preferred.
- Current, unrestricted RN license in the state(s) of practice.
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
- One (1) year of supervisory and/or case management experience preferred.
- Current CPR certification.
- Competitive hourly rate of $40.00, plus shift differentials where applicable.
