Key Responsibilities:
  • Conduct in-home nursing visits to assess clients' physical and emotional needs
  • Develop and implement personalized care plans to meet clients' unique needs
  • Collaborate with clients, families, and healthcare teams to ensure seamless care coordination
  • Supervise and train caregivers to ensure high-quality care
  • Document client progress and care plans in a timely and accurate manner
Requirements:
  • Graduate of a state-approved school of professional nursing
  • Current, unrestricted RN license in the state(s) of practice
  • Minimum of two years of nursing experience, including one year in home care or a closely related field
  • One year of supervisory and/or case management experience preferred
  • Current CPR certification
