Nurse Case Manager

At Professional Case Management, we are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager. As a key member of our team, you will have the opportunity to make a meaningful difference in the lives of our clients, who are former Nuclear Weapon's Workers with chronic/terminal illness due to exposure to toxic substances while working for the Government.
Key Responsibilities:- Conduct in-home nursing visits for our clients, providing physical assessments, ensuring home safety, and overseeing skilled and non-skilled caregivers.
- Provide case management to assigned clients, directing a team of RNs, LPNs, and caregivers in the delivery of care.
- Collaborate with the regional director and other personnel to identify and correct issues, improve services, and ensure client needs are met.
- Plan, implement, and evaluate care provided, participating in client care conferences as needed.
- Graduate of a state-approved school of professional registered nursing.
- BSN preferred.
- Current, unrestricted RN license in the state(s) of practice.
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
- One (1) year of supervisory and/or case management experience preferred.
- Current CPR certification.
- Competitive hourly rate of $40.00, plus shift differentials where applicable.
