Nurse Case Manager

We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team at Professional Case Management. As a key member of our team, you will be responsible for providing high-quality care to our clients in their homes.
Job Summary:The Registered Nurse Case Manager will conduct in-home nursing visits, assess clients' needs, and develop personalized care plans to ensure their optimal health and well-being. You will work closely with our clients, their families, and our interdisciplinary team to provide comprehensive care and support.
Responsibilities:- Conduct in-home nursing visits to assess clients' physical and emotional needs
- Develop and implement personalized care plans to meet clients' unique needs
- Collaborate with clients, families, and healthcare providers to ensure seamless care coordination
- Provide education and support to clients and their families on disease management, medication adherence, and healthy lifestyle habits
- Monitor clients' progress, adjust care plans as needed, and communicate with the healthcare team
- Participate in case conferences and contribute to the development of care plans
- Ensure compliance with regulatory requirements and agency policies
- Graduate of an accredited nursing program
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two years of nursing experience, including one year in home care or a related field
- One year of supervisory and/or case management experience preferred
- Current CPR certification
- Competitive hourly rate: $40.00 per hour, plus shift differentials
