About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Medicare Wellness RN. In this role, you will play a vital part in providing exceptional care to our patients, focusing on their overall health and well-being.

Key Responsibilities
  • Provide annual Medicare wellness visits for FPG patients, ensuring a comprehensive and personalized approach to their care.
  • Assist in pre-visit planning and identification of eligible patients, streamlining the process for optimal outcomes.
  • Maintain up-to-date knowledge of Medicare requirements and policy updates, ensuring compliance and adherence to regulatory standards.
  • Collaborate with the care management team to refer patients for care coordination and health coaching when necessary, promoting a seamless and patient-centered experience.
  • Establish individualized, objective action steps with patients, working together to achieve their health goals and improve overall well-being.
  • Provide ongoing support and evaluation of progress, revising healthcare plans as needed to ensure the best possible outcomes.
  • Facilitate patient medication management based on standing orders and protocols, ensuring safe and effective care.
  • Act as a patient advocate, empowering them to achieve their maximum level of health and well-being.
  • Deliver education to patients about their specific conditions and needs, promoting informed decision-making and self-care.
  • Possess excellent communication and collaboration skills, fostering strong relationships with patients, medical staff, and administrative teams.
  • Work collaboratively with primary care providers to coordinate referrals, testing, and closure of gaps in care, ensuring a cohesive and patient-centered approach.
  • Maintain accurate and timely EHR documentation, supporting data-driven decision-making and quality improvement initiatives.
  • Participate in medical home model of care and value-based care initiatives, driving excellence in patient care and outcomes.
  • Contribute to a positive and supportive work environment, interacting harmoniously with colleagues and promoting a sense of well-being within the office setting.
  • Focus on achieving organizational goals and objectives, demonstrating a commitment to teamwork and collaboration.
  • Be flexible and adaptable, assisting the care team with other patient care duties as assigned, ensuring seamless delivery of care.
  • Participate in Care Management team meetings and quality improvement initiatives, driving continuous improvement and excellence in patient care.
  • Contribute to data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to Medical Home initiatives, supporting evidence-based practice and quality improvement.
  • Evaluate clinical care, utilization of resources, and development of new clinical tools, forms, and procedures, ensuring best practices and continuous improvement.
  • Label, document, and track all specimens or tests ordered/received, following established standards and reporting test results to physicians.
  • Chart care coordination on the day of visit, ensuring a documented, organized, and chronological medical record with timely, accurate, and complete information submitted to the Central Billing Office.
  • Assist with scheduling appointments, tests, or procedures, ensuring seamless coordination of care.
  • Answer phones in a pleasant manner, screening calls according to clinic guidelines to direct callers appropriately and expeditiously.
Requirements
  • Graduate of an accredited program of professional nursing and possess a current license as a Registered Nurse from the State of Ohio.
  • Currently certified in BLS and maintain certification.
  • 3 years experience in a clinical setting required, with chronic disease management and adult preventive care knowledge demonstrated.
  • Proficient computer skills, including Microsoft Office (specifically Word and Excel).
  • Highly organized and well-developed oral and written communication skills.
  • Demonstrates sound judgment, decision-making, and problem-solving skills.
  • Interpersonal skills to develop effective relationships with medical and administrative staff, patients, co-workers, and the public.
  • Ability to remain calm and poised in urgent situations.
  • Skill in exercising initiative, judgment, problem-solving, and decision-making.
  • Valid State of Ohio motor vehicle operator's license and insurable under Firelands auto policy.
  • Ability to work in a safe and efficient manner, maintaining an accident-free work place, including knowledge of emergency codes.
  • Compliance with OSHA regulations and CDC standards, utilizing proper personal protective equipment.
  • Compliance with provisions of applicable S.D.S. forms.
  • Successful completion of a 90-day get-acquainted period.
