Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Medicare Wellness Nurse. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and promoting health and wellness in our community.

Responsibilities
  • Provides annual Medicare wellness visits for patients, ensuring timely and accurate completion of required documentation.
  • Assists in pre-visit planning and identification of eligible patients, utilizing clinical knowledge and expertise to optimize patient outcomes.
  • Maintains compliance with Medicare requirements and policy updates, staying up-to-date on the latest guidelines and regulations.
  • Collaborates with the care management team to coordinate patient care and refers patients for care coordination and health coaching as needed.
  • Establishes individualized, objective action steps with patients to improve their overall health, working collaboratively with the patient and healthcare team to achieve goals.
  • Provides ongoing support and evaluation of progress toward stated health goals, revising the healthcare plan as needed in collaboration with the patient and healthcare team.
  • Facilitates patient medication management based on standing orders and protocols, ensuring safe and effective medication use.
  • Acts as an advocate for patients, helping them to obtain their maximum level of health and well-being.
  • Provides education to patients about their specific conditions and needs, utilizing effective communication and interpersonal skills.
  • Collaborates with the primary care provider to coordinate needed referrals, testing, and closure of gaps in care, ensuring seamless patient care.
  • Maintains accurate and timely Electronic Health Record (EHR) documentation, adhering to organizational standards and policies.
  • Participates in medical home model of care and value-based care initiatives, contributing to the development and implementation of quality improvement projects.
  • Interacts harmoniously with colleagues and contributes to a positive and supportive work environment, promoting a sense of well-being and teamwork.
  • Focuses on the attainment of organizational goals and objectives, demonstrating a commitment to excellence and customer satisfaction.
  • Is flexible and willing to assist the care team with other patient care duties as assigned, ensuring seamless patient care and efficient use of resources.
  • Participates in Care Management team meetings and quality improvement initiatives, contributing to the development and implementation of best practices.
  • Participates in data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Medical Home initiatives, ensuring accurate and timely reporting.
  • Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures, contributing to the improvement of patient care and outcomes.
  • Labels, documents, and tracks all specimens or tests ordered/received, following established standards and reporting all test results to the physician.
  • Charts care coordination on the day of visit, ensuring a documented, organized, and chronological medical record with timely, accurate, and complete information submitted to the Central Billing Office.
  • Assists with scheduling appointments, tests, or procedures, ensuring efficient use of resources and timely completion of tasks.
  • Answers phones in a pleasant manner, screens calls following clinic guidelines to appropriately and expeditiously direct the caller.

  • Practices Human Resource Responsibility, adhering to organizational policies and procedures.
  • Demonstrates competence in leadership, communication, professionalism, organizational, and analytical skills, and technical/professional skills and knowledge in performance of duties and responsibilities rendered to the customer population served.
Requirements
  • Graduate of an accredited program of professional nursing and possess a current license as a Registered Nurse from the State of Ohio.
  • Currently certified in BLS and maintain certification.
  • 3 years experience in a clinical setting required with chronic disease management and adult preventive care knowledge demonstrated.
  • Proficient computer skills, including Microsoft Office (specifically Word and Excel).
  • Highly organized and well-developed oral and written communication skills.
  • Demonstrates sound judgment, decision-making, and problem-solving skills.
  • Interpersonal skills such that employee can develop effective relationships with medical and administrative staff, patients, co-workers, and the public.
  • Ability to remain calm and poised in urgent situations.
  • Skill in exercising initiative, judgment, problem solving, and decision making.
  • Must possess a valid State of Ohio motor vehicle operator's license and insurable under Firelands auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
  • Ability to work in a safe and efficient manner and maintain an accident-free work place, including ability to demonstrate a working knowledge of emergency codes.
  • Ability to comply with OSHA regulations and CDC standard and transmission-based precaution recommendations and to utilize proper personal protective equipment.
  • Ability to comply with provisions of applicable S.D.S. forms.
  • Successful completion of a 90-day get-acquainted period.
