Position: Care Coordination Manager
Role Overview:
As a dedicated and passionate leader in nursing, you will play a pivotal role in enhancing patient care through effective coordination and management. Your commitment to building meaningful relationships with your team, patients, and their families will be essential in fostering a compassionate healthcare environment.
Key Responsibilities:
- Facilitate collaboration among multidisciplinary teams to ensure safe and high-quality patient transitions from admission to discharge.
- Act as a key point of contact between various patient care services to optimize bed assignments and enhance the overall patient experience.
- Lead regular unit meetings aimed at identifying and resolving obstacles to effective patient care plans.
- Engage in escalation discussions, highlighting any discharge barriers to relevant stakeholders.
- Serve as a clinical and technical mentor for staff, integrating your expertise into daily practices.
Qualifications:
- Strong passion for Care Coordination.
- Minimum of 1 year of Case Management experience as a Registered Nurse (RN).
- At least 1 year of leadership experience in nursing.
- Education: Bachelor of Science in Nursing (BSN) degree.
- Licensure: Valid New York Registered Nurse (RN) License & Registration.
- Certification: Basic Life Support (BLS) certification.
