JOB SUMMARY
At Houston Methodist Clear Lake Hospital, the Clinical Nurse Specialist for Wound Care plays a pivotal role in delivering expert, integrated, and cost-effective patient care. This position focuses on coordinating clinical services for high-risk patients facing complex health challenges across various care settings. As a key clinical resource and consultant, the role addresses acute and rehabilitative care for patients with specific disorders affecting the gastrointestinal, genitourinary, and integumentary systems. Responsibilities include managing wounds, ostomies, fistulas, dermal ulcers, tubes, and continence disorders. The Clinical Nurse Specialist oversees wound treatment plans and engages in performance improvement initiatives. This role requires a thorough understanding of hospital policies, procedures, and regulatory standards. The nurse clinician integrates advanced knowledge of pathophysiology and human responses to health issues to assess, plan, coordinate care, and evaluate patient outcomes. The position demands the ability to interpret data reflective of the patient's condition and respond to age-specific needs. Active participation in a collaborative practice framework is essential.
PRIMARY JOB RESPONSIBILITIES

- Serves as a clinical resource and consultant for specialty population care by collaborating with staff, management, patients, families, and other relevant groups. Identifies opportunities for professional development among colleagues and provides coaching to achieve identified goals. Models effective communication strategies regarding specialty populations, including wound, ostomy, and continence needs.
- Utilizes the nursing process to direct and deliver care to patients with wound, ostomy, and/or continence needs. Makes informed recommendations based on patient status, safety, and developmental needs, intervening as necessary to support staff, patients, families, and physicians.
- Facilitates interprofessional planning to ensure patient safety and quality care, addressing clinical concerns raised by physicians.
- Exemplifies patient and family-centered care standards as outlined in the Houston Methodist Professional Nursing Practice document. Acts as an advocate for patients by coordinating the interprofessional team to provide holistic, goal-oriented care that meets the physical, psychosocial, and spiritual needs of patients. Practices in alignment with principles of cultural diversity and inclusion.
- Develops and implements effective patient education strategies tailored to individual learning needs, utilizing appropriate resources and planning for post-discharge care. Consistently assesses patient comprehension and adapts teaching methods as needed, applying evidence-based knowledge to recommend changes in care plans.
- Models care standards related to the nursing process as outlined in the Houston Methodist Professional Nursing Practice document. Participates in the development, implementation, and evaluation of nursing care standards affecting practice in assigned specialty areas.
- Adheres to regulatory requirements and professional practice standards, utilizing expertise in skin integrity, wound, ostomy, and continence care to establish and update care standards. Implements nursing actions to maintain optimal clinical outcomes.
- Documents accurately in the electronic medical record and reviews interprofessional documentation for validation.
- Oversees and conducts Pressure Injury Prevalence surveillance in collaboration with Quality and Patient Safety, developing action plans based on findings.
- Demonstrates effective time management and prioritization of referrals and treatments.
- Contributes insights on wound care and ostomy products, equipment, and support surfaces, balancing quality patient care with cost considerations. Ensures the judicious use of resources for effective patient care.
- Drives efficiencies and quality improvements while recognizing resource limitations, managing programs within established timeframes and budgets.
- Develops proposals for new programs, projects, and nursing research initiatives. Encourages innovative solutions to enhance patient care and unit operations, such as reducing hospital readmissions, by leading or participating in hospital projects and shared governance activities.
- Identifies opportunities for practice changes and performance improvements, incorporating evidence-based practices with input from shared governance and leadership as appropriate. Supports change initiatives and adapts to unexpected changes.
- Continually expands nursing knowledge, maintaining and updating the individual development plan (IDP). Coaches staff to enhance their knowledge, skills, and competencies. Serves as a clinical resource and develops educational resources aimed at improving quality and professional practice.
EDUCATION REQUIREMENTS
- Bachelor's degree from an accredited nursing program
- Master's degree preferred
- Two years of clinical nursing experience in a specialty or related area
CERTIFICATES, LICENSES AND REGISTRATIONS REQUIRED
- Texas RN license or temporary TX RN license, with the expectation of obtaining a permanent license within 90 days. Compact license acceptable according to current Board of Nursing requirements.
- American Heart Association Basic Life Support (BLS) certification.
- American Nursing Credentialing Center (ANCC)-recognized Wound and Ostomy certification or related area within six months of eligibility.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
- Demonstrates the skills and competencies necessary to safely perform assigned duties, assessed through ongoing skills evaluations and performance reviews.
- Proficient in speaking, reading, and writing the English language to perform essential job functions effectively.
