Your Mission:

As a Case Manager for the Illinois Welcoming Center, you will play a crucial role in assisting clients assigned by the Community Services Coordinator. Your primary responsibility will be to identify client needs using the Social Determinants of Health (SDoH) framework during the intake process, link clients to essential services, and follow up on all referrals to ensure comprehensive support.

Your Impact:

As a Case Manager for the Illinois Welcoming Center you will:

Key Responsibilities:

Culturally Sensitive Support: Provide case management and service coordination that respects the cultural and linguistic needs of immigrants and refugees and their families.

Emergency Services: Offer immediate crisis intervention and support through thorough intake assessments that inform service plans.

Service Coordination: Connect clients with necessary services and ensure they receive the support needed by following up with internal or external providers.

Interdepartmental Collaboration: Participate in meetings with relevant staff to discuss and follow up on referrals and support not provided by CASL.

Community Engagement: Engage in workshops and Community Alliance meetings related to the Illinois Welcoming Center.

Data Management: Accurately collect and enter client information into Salesforce and other funder database systems as required.

Confidentiality: Maintain client information and files in a professional, accurate, and confidential manner.

Professional Development: Attend relevant training sessions to enhance your skills and knowledge.

Agency Support: Perform additional tasks as needed to support the agency's mission.

Must Haves

To thrive in this role, you should have:
  • Demonstrated experience in client-facing roles, ensuring high levels of client engagement and exceptional client service, minimum of 2+ years of experience
  • Spoken and written knowledge of English and Chinese Cantonese highly preferred, Mandarin preferred, ability to speak both highly preferred)
  • Proficiency with computer applications inclusive of Microsoft office.
  • Knowledge of immigration laws and the naturalization process.
