The Early Intervention Service Coordinator is responsible for advocating, planning, coordinating, and monitoring services and supports provided to families of infants and toddlers eligible for Part C Early Intervention services from an assigned caseload of children. The Service Coordinator will facilitate initial and ongoing Individualized Family Service Plan (IFSP) meetings, complete family assessment, support the family to ensure services are being delivered in accordance with the IFSP, monitor the provision of services, supports, and recommendations identified in the IFSP, assist in obtaining identified services and supports, maintain a master record and all necessary documentation, serve as the primary contact for obtaining and disseminating information, and facilitate and mediate conflict resolution. Inherent in all these functions shall be activities supporting families Rights and Procedural Safeguards under Part C, support, empowerment, and advocacy. Must have a bachelor's level degree of education; or Five years of experience in the field of developmental disabilities; or some combination of education and experience appropriate to the requirements of the position.
