Shift: Mon - Fri (8am - 5pm)

The Care Transition Coordinator will work full time; floating between the different Hospice & Home Care offices in the Hampton Roads area.

The Care Transition Coordinator is responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.

Requirements:

  • 1 year of Home Care/Hospice, Discharge Planning OR Case Management experience required
  • Licensed RN, LPN, RT or BSW required

Job Summary

Responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.

1 year of home care, discharge planning or case management experience required. Licensed RN, LPN, RT or BSW required. Proof of education as LPN, RN, RT or BSW required.
