Position Overview:
As an Interim MDS Coordinator at Autumn Care of Chesapeake, you will play a crucial role in managing the Resident Assessment Instrument (RAI) process, ensuring compliance with both State and Federal regulations as well as the community's policies and objectives.

Key Responsibilities:
- Conduct comprehensive assessments of residents to facilitate quality care.
- Collaborate with healthcare personnel, residents, and families to address care needs.
- Develop, implement, and interpret care programs and policies.
- Maintain detailed records and documentation regarding residents' conditions.

Qualifications:
- Must hold an active, unencumbered Registered Nurse license in the state of practice.
- Demonstrated knowledge of nursing practices and regulations pertinent to long-term care facilities.
- Ability to effectively communicate with diverse groups including staff, residents, and families.
- Strong organizational skills and attention to detail are essential.

Experience:
- A minimum of one year of experience with the RAI process is preferred.

Work Schedule:
- Full-Time positions available with a focus on day shifts.
