Duties/Responsibilities: Perform monthly visitations to Public Guardian clients, and accompany clients to medical appointments. Provide case management services to all Public Guardian clients. Assist with daily operations of Public Guardianship program, clients, and volunteers. Create and utilize spreadsheets for periodic reports used for tracking and reporting client data. Maintain database of accurate client information and document management. Provide information and assistance for state monitoring and assessments of the program. Knowledge of the laws and regulations pertaining to the elderly population and to the delivery of services as established under the Public Guardianship for the Elderly Law. Good organizational, interpersonal, decision-making skills, and dedicated attention to detail; or Any equivalent combination of education and/or experience. Valid TN driver license.
