Summary:
The Case Management Assistant, under the direction of the Manager of Case Management assists in the administration of Case Management functions to include, but not limited to, organization of work flow, communications, delivery of regulatory forms, data entry, and links with community and transitional care resources. The position is focused on supporting the role of the Case Management department in optimizing the care management process and case manager efficiency and effectiveness.
Communicates and documents accurate clinical and regulatory information in both verbal and written forms in order to assist Case Manager and/or Social Worker in appropriate assessment, planning, implementation and evaluation of the patient's continuing care needs.
Supports the Case Management outcome model through clinical, clerical and support services in order to assist with the planning, assessment and implementation of appropriate patient transitions.
Job Responsibilities:
1) Exceptional organization skills; ability to communicate effectively with others; ability to work well on a multidisciplinary team; and excellent time management skills.

3) Exceptional interpersonal skills and ability to interact and communicate with patients, families, and community resources.
4) Capability of assisting with follow-up appointments and telephone calls, and other post acute/transitional planning care needs.
5) Knowledge/willingness to learn baseline Medicare and Medicaid guidelines and eligibility criteria.
6) Supporting coordination of care activities under the direction of a RN case manager and/or social worker as needed.
7) Supporting coordination with representatives from insurance companies, accountable care organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies.
8) Assists in collecting data related to patient outcomes and auditing data for accuracy.
9) Collaborates with internal and/or external partners to address financial advocacy for patient/organization.
10) Issuance of CMS Important Message/MOON/Regulatory notices to patients/designee.
11) Potential to cross train for Case Management Coordinator / Denials & Appeals Coordinator
12) Potential to participate in quality improvement initiatives per the needs of department.
