JOB SUMMARY:

The Case Manager is responsible for the coordination and delivery of quality direct case management and stabilization services for clients in the Victory’s Path, Veterans Housing Program in accordance with standards and practices required by the agency and the Veteran Administration Program. The Case Manager is also responsible for providing housing advocacy and linkage services that help to support retention in housing and within two years allow the client to transition in place and assume lease responsibility.

JOB RESPONSIBILITIES:
  • Provide ongoing case management services to program participants by:
  • Working collaboratively to assist the clients/residents obtain resources, including employment or/and benefits that will allow them to assume the lease in two years or less.
  • Documenting all in-person and phone contacts with program participants and maintain quality case notes.
  • Developing and monitor service plans as required by agency standards and funding sources.
  • Developing and maintain service linkages to support clients/residents and their families, including developing relationships with local service providers.
  • Connecting participants to medical, mental health and other supportive services as needed.
  • Transport clients or arrange for transportation to housing, legal, and medical appointments as needed.
  • Providing ongoing life-skills training including problem solving and coping/ skill development to aid in the participants’ process of improving independence and quality of life.
  • Establishing a broad-based knowledge of community services and communicating appropriate referrals to clients.
  • Conducting visits in the client’s home to support client in working on goals, to ensure compliance with lease and program rules.
  • Assuring access and continued eligibility to appropriate entitlements in which participants may be eligible.
  • Participating in on-site and off-site trainings and meetings, as needed.
  • Working as part of a team to ensure service delivery to all members of Supportive Housing Program.
  • Participating in clinical case conferences and care coordination as indicated.
Program recordkeeping and compliance
  • Meeting regularly with staff from Hines VA to receive referrals and monitor clients’ progress.
  • Complete all internal and external designated reports/databases and maintain effective documentation and record keeping including the logs, client case files, and other programmatic needs accurately and in a timely manner as directed by the supervisor.
  • Prepare for and participate in the monthly/quarterly monitoring visits by the VA staff.
Additional Duties:
  • Work with internal and external source to place clients in housing as soon as possible. this includes outreach to identified client from the street, Interim Housing, VA hospital community organization etc.
  • Participate in the Tenant Club (Independent Living Skills Group) by encouraging participation of assigned clients and facilitate group as indicated.
  • Work with Housing Navigator to place client in appropriate housing. Once placed, work with Housing Navigator to resolve any landlord-tenant conflict.
  • Ensure that clients that are able; pay appropriate program fees.
  • Take the lead on at least one team project per year (ILS Group, House to Home, file Monitoring, etc.)
  • Facilitate Housing Team Meeting on a rotating basis.
  • Other duties as assigned.
QUALIFICATIONS:
  • Bachelor’s degree in the area of social service.
  • Master’s degree preferred.
  • Word processing, data entry and spreadsheet skills.
  • Bi-lingual preferred.
  • Must provide proof of COVID-19 vaccination.
  • Must have smartphone to utilize Agency communication and collaboration tools.
  • Driver's license and personal transportation is necessary for this position.
KEY COMPETENCIES:
  • Excellent case management skills, ability to work independently and as part of a team.
  • Demonstrates experience and ability to interact sensitively and respectfully with persons with severe disabilities (mental health, substance use or other disability condition) and homeless difficulties. Knowledge of and interest in trauma informed care, housing first and harm reduction theory.
  • Demonstrates strong skills in interviewing, engagement, assessment, planning and advocacy.
  • Demonstrates good judgment, and ability to work independently.
