We are searching for a talented Community Health Worker

Hours: 40hrs

Shift: Day/evening Shifts; 8:30am - 5:00pm

Location: Southcoast Business Center - Fairhaven, MA

Travel: Driving on average 3 or more times per week required

Responsibilities

Under the general direction of the Director, Clinical Operations - Chart 2 or designee, in collaboration with a multidisciplinary care team, provide culturally appropriate health education, coaching, support, information and outreach in a variety of settings including community-based, primary care, ambulatory and acute care settings as needed. Identify high risk patients, associated health needs. Facilitate the coordination of community resources to meet the patient-s plan of care. Assist patients to practice healthy behaviours and access necessary social and medical support services and programs. Conduct home and community visits.

Qualifications
  • Equal to completion of 4 years of high school plus additional specialized courses or training is required.
  • Bilingual skills preferred.
  • Two years experience in community based member outreach.
  • Proven ability for independent decision making and problem solving while performing effectively as part of a multi-disciplinary team.
  • Solid knowledge of community resources and effective teaching/coaching skills.
  • Verifiable good driving record and reliable transportation.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.


Pay Range

USD $21.62 - USD $36.36 /Hr.
