We are seeking a dedicated Community Health Worker to join our Bridges to Moms Program at Brigham and Women's Hospital. As a Community Health Worker, you will play a vital role in supporting Obstetrics patients who are experiencing homelessness or are at risk of becoming homeless.
Key Responsibilities:
- Provide comprehensive care and assistance to patients throughout the baby's first year
- Close gaps in community resources related to Social Determinants of Health (SDOH) needs
- Collaborate with patients and providers to set and achieve health goals
- Offer culturally sensitive services to patients from diverse backgrounds
- Help patients manage their care effectively by developing support systems
- Assist with logistical barriers to care such as scheduling and transportation issues
- Accompany patients to appointments and advocate on their behalf
- Communicate regularly with providers and care teams
- Refer patients to additional services when needed
- Assist patients in filling out applications for public services
- Contribute to program reports and attend training programs
Requirements:
- Minimum two years of relevant work experience or Associates Degree
- High school degree required
Preferred Qualifications:
- Knowledge of community-based services and resources
- Excellent communication skills
- Ability to connect authentically with individuals from diverse backgrounds
- Strong attention to detail and organization
- Willingness to learn service approach models
- Strong time management and planning skills
- Ability to collaborate with multidisciplinary teams
- Proficiency in computer skills
- Fluency in English and Spanish or Haitian Creole
