POSITION SUMMARY

The Community Center Case Manager carries a caseload of 30 Seniors and provides case management support individually and in groups to St. Mary's Center members to meet an individual's health, financial, social, and other senior needs to maintain housing. Case management includes assistance either in the form of access or care coordination where a client is experiencing diminished functioning capacities, personal conditions or other characteristics.

Case management (60%)

  • Maintain a caseload of 30 enrolled clients and provide professional and culturally appropriate assessment, diagnosis, and treatment planning as well as advocacy, information and referrals for acutely and extremely low income and at risk seniors.
  • Assess, plan, implement, coordinate, monitor, and evaluate services to address an individual's health, financial, and social needs via home visits and phone calls.
  • Offer daily office hours for drop-in services to seniors to support light touch needs.
  • Link clients to community social services, including physical and mental health services, financial benefits, legal services, substance use treatment and daytime activities at St. Mary's Center or other appropriate locations.
  • Provide crisis support to clients experiencing acute symptoms.
  • Facilitate transition to higher level of care such as Board and Care and Skilled Nursing Facilities when necessary.
  • Maintain clinical records in agency database as well as County systems in accordance with mandated standards of care for St. Mary's Center and/or Alameda County Behavioral Health.
  • Maintain confidentiality regarding patient health information and comply with HIPAA requirements.
Collaboration (20%)
  • Work within and promote an environment dedicated to harm reduction, recovery, and wellness, setting an effective model of de-escalation, client support and avoidance of re-traumatization.
  • Maintain effective communication with Senior Housing Services team to support transition from homelessness to housing and participation in Community Center Activities.
  • Collaborate with Community Services Manager to deliver educational, recreational and social activities that address Senior needs for companionship and community, including regular events, annual picnic, informational workshops on benefits and programs.
  • Represent agency at designated governmental, funding or training meetings, and develop relationships with partner agencies for adopting best practices and accessing additional services from community-based providers, including language access.
Documentation (20%)
  • Maintain accurate and timely clinical documentation in accordance with agency, regulatory, and ethical standards.
  • Complete progress notes, treatment plans, and discharge summaries, as well as other required documentation in St. Mary's Center's internal database and external databases.
  • Ensure compliance with HIPAA regulations and confidentiality standards.
  • Attend required trainings and compliance activities.
Skills and Qualifications
  • Knowledge of age-specific theories of human development, social work theory and practice as well as applicable community resources and legal aspects of care for housed and unhoused seniors.
  • Knowledge of mandated reporting laws and legal rights and responsibilities.
  • Familiarity and understanding of supportive housing strategies that foster retention, stabilization, and maintenance of housing, health and wellbeing.
  • Knowledge of relevant clinical practices, like trauma informed care, motivational interviewing and cultural humility to support seniors in aging with dignity.
  • Minimum of 3 years experience working with people experiencing homelessness.
  • Must be able to work collaboratively within a strong team environment, and manage designated tasks independently.
  • TB test required, flu shot recommended.
  • Written and verbal language capacity in English, Cantonese, Mandarin or Tagalog is a plus.
