The Case Manager plays a crucial role in our Media Education and Employment department, providing clinical case management support and coordinating wraparound services for transition-aged youth (14-24 years old). This part-time position involves intake, assessment, crisis response, and ongoing care coordination, including developing individualized wellness plans and facilitating referrals to internal and external services. The Case Manager will work on-site with youth enrolled in our media education programs and interns in our digital technology training program, ensuring their well-being and development. Additionally, this role includes managing and maintaining YR's food pantry. This position reports to the Program Site Manager.

JOB RESPONSIBILITIES:
  • Core Results and Accomplishments:
    • Provide effective on-site tiered case management for YR Media participants, including administering biopsychosocial assessments and developing individualized action plans.
    • Monitor and support youth participation in YR Media's education programs, ensuring successful outcomes and contributing to their holistic development.
    • Collaborate with external stakeholders (parents/guardians, teachers, social workers, etc.) to support youth development and maintain detailed and confidential records on all participants.
    • Manage and maintain YR's food pantry to support youth needs.
    • Navigate the complexities of supporting diverse youth in a dynamic environment.
    • Manage time efficiently to balance multiple responsibilities and meet deadlines.
    • Adapt to changing needs and address unforeseen issues promptly and effectively.
    • Proactive engagement with youth and staff, including participation in meetings, trainings, and workshops.
    • Align with YR Media's mission to empower young BIPOC content creators and drive systemic change through media education and holistic support.
    • Contribute to the overall success of the Media Education and Employment program by providing comprehensive support services and fostering a supportive environment for learning and development.
Requirements
  • Knowledge, Skills, and Experience:
    • Proven experience in case management, including intake, assessment, crisis intervention, and resource utilization.
    • Ability to quickly adapt to new technology, automated systems, and databases.
    • Experience supporting system-engaged or previously system-engaged youth in achieving goals.
    • Strong organizational skills to manage documentation, progress notes, incident reports, and grant reporting data.
    • Ability to work independently, prioritize tasks, and handle a variety of responsibilities.
    • Clinical license as LCSW or MFT required (intern-level working toward licensing hours acceptable).
    • Bachelor's degree in social work, psychology, public health, counseling, or equivalent education and experience required. Master's degree preferred.
    • Minimum of 3-5 years of direct service experience with youth.

Salary Description

$34.00/hr. - $36.00/hr.
