Lao Family Community Development, Inc. (LFCD) is a community development non-profit agency established in the City of Richmond in Contra Costa County in 1980. Today Lao Family has expanded its operations and service footprint to two additional counties including Alameda and Sacramento. LFCD’s headquarters office is in Oakland, CA. It delivers programs and services from 7 locations in 35 languages. The agency provides both community development real estate facilities and a diverse array of the workforce, education, and human service that directly support predominantly low-income US-born high-barrier families and individuals, refugees, immigrants, transitional-age youth, seniors, and other special populations such as individuals with disabilities.


Job Summary: Under the direction of the Program Coordinator and/or Site Supervisor, this position will manage participants enrolled in the Cal-AIM program. This program intends to provide comprehensive care management that addresses clinical and non-clinical needs for those with the highest needs through intensive coordination of health, community, and social services that will meet the participants wherever they are - at home, in the hospital, or the doctor’s office, or in alternative housing settings such as on the street or in the shelter. The Case Manager will coordinate care, assess the participant’s health condition, and provide them with the necessary resources for a healthy care outcome.


Note: This position is intended for Lao Family’s Oakland office. However, for training purposes, the first 3 months of this position will take place in our San Pablo office. This position is expected to transition back to the Oakland office as early as January 1, 2025.


Roles and Responsibilities:

  • Conducts individual assessments to develop appropriate personalized care management plans that address physical health, mental health, substance abuse disorder(s), community-based support services, oral health, palliative care, social supports, and/or other social determinants of health.

  • Provide outreach and engagement to the community; coordinate referrals to community and social services providers to achieve goals and objectives of the participants' care management plan.

  • Coach participants to make lifestyle choices that promote healthy behaviors through 1:1 interactions and in workshop settings.

  • Scheduling appointments, appointment reminders, coordinating transportation, accompanying clients to critical appointments, and identifying and addressing other barriers to participant’s engagement in their care management plan

  • Network with stakeholders and community organizations to remain current on issues and activities that can potentially impact the program and the services.

  • Create and implement educational workshops that encourage healthy living, independence, and goal-setting skills to enhance their well-being.

  • Assist participants in accessing additional benefits and related documentation such as Social Security Insurance (SSI), Advance Directive, CalFresh, cash aid, and obtaining the required documentation to apply (ID, birth certificate, immigration status, financial records, marriage/divorce records, proof of medical conditions, etc.);

  • Provide transitional care to participants during discharge from the hospital or institutional setting

  • Develop and maintain detailed knowledge of the CalAIM program including the cultivation of information and strategies gleaned through partnerships and networks within the healthcare space.

  • Create and maintain case files; maintain accurate documentation regarding all information received and actions taken; comply with HIPAA regulations.

  • Maintain accurate logs and compile records of work performed.

  • Provides administrative and programmatic support to the Director, Managers, and/or Site Supervisors as needed

  • Receives, sorts, analyzes, and summarizes material for the preparation of case files; reports; and maintenance of records.

  • Prepare advanced word processing, spreadsheet, and presentation documents as assigned including manuals, brochures, reports, and PowerPoint presentations.

  • Other duties as assigned.

Requirements and Qualifications:

  • Bachelor's Degree highly desired; concentration in social work, psychology, communications, or other related fields. If no degree, a minimum of 1 year of case management experience working with sensitive needs populations including but not limited to low-income, unhoused, unemployed, youth, justice-involved community members, English as a second language, etc.

  • Self-starter, ability to work with minimal supervision; excellent communication, multi-task, community relations, networking, and public speaking skills.

  • Demonstrated ability to work with families, women, and children without discrimination towards people of diverse cultures, races/ethnicities, socio-economic positions, ages, religions, genders, physical/mental challenges/disabilities, and sexual orientations.

  • Understand, explain, and apply complex local, state, and federal regulations, policies, and procedures.

  • Able to travel to meet with service providers and participants; ability to work independently as well as part of a team; must have a flexible schedule on some evenings and weekends.

  • Proficient computer skills in MS Word, Excel, PowerPoint, database management, and internet explorer.

  • Must have a valid California Driver's License, clean DMV record, and proof of current auto insurance and registration; must pass a background check. Compensation: Hourly wage based on experience and education, along with a comprehensive benefits package including health plan/vision, dental, paid vacations, holidays, sick leave, and employer-contributed pension/group life insurance.
