The Hamilton County Educational Service Center has an opening for a Part Time Transition Coordinator

QUALIFICATIONS:
ODE Intervention Specialist Teaching License with Transition to Work Endorsement preferred

REPORTS TO:
Director of Intervention and Support Services Supervisor, or designee

JOB SUMMARY:
To provide transition services to district staff, students, and families that are designed within an outcome-oriented process, that promotes movement from school to post-school activities, including post-secondary education, vocational training, competitive integrated employment, continuing and adult education, adult services, independent living, or community participation.

ESSENTIAL FUNCTIONS:
1. Assist school staff in developing individualized instruction and activities for students in transition areas of employment/post-secondary school/adult living, community participation, under IDEIA rules and regulations.
2. Provide age-appropriate transition assessments to assist teams in collecting information for transition plan development.
3. Participate in team meetings, planning meetings, and IEP meetings to develop transition plans, goals, objectives, and services. May include writing appropriate and IEP goals, objectives, and services in areas of transition.
4. Contact employers to develop job shadow activities, job experiences, work training sites, and employment sites in the community, career tours and provide student supervision and transportation to those locations.
5. Arrange, transport, and supervise tours of various programs including but not limited to career and technical schools, day programs, transition programs, and adult living options.
6. Assign and train Job Coaches (or School Staff who may provide student support) on job sites. Monitor progress of staff and students.
7. Attend and participate in problem solving meetings with staff to design individualized interventions to address student challenges on specific job sites. Provide written interventions and training to Job Coach to support designed intervention and progress monitoring for student.
8. Complete or coordinate documents for students who will be on a community job site (Task analysis of job/data collection sheet for job performance, work site agreements, work permits, job applications, resumes, etc.).
9. Work with district personnel (teachers, counselors, case managers) and outside agencies to provide parents with linkages to supports for post school (including adult services, job development and follow along services, etc.).
10. Learn and utilize technology for a variety of purposes.
11. Obtain necessary training and updates for Van Driver's License (ODE rules) in order to transport students. Hold a valid Ohio Driver's License and provide own transportation. Must be able to travel to various locations on a daily basis.
12. Keep current in labor laws, job market changes, transportation linkages, Federal/State Rules/Regulations and HCESC policy and procedures.
13. Keep current with available post school adult services (i.e., OOD, DDS, day programs, etc.), to assist students, families, and staff in knowing all options.
14. Adheres to ethical and professional practice standards and guidelines as defined by appropriate professional organizations.
15. Protects confidentiality in all situations related to professional practice.
16. Demonstrate positive conflict resolution skills in verbal and written communications.
17. Communicates with supervisor and/or other professionals regarding common problems and/or legal and ethical concerns.
18. Participates in yearly professional growth and development through coursework and/or seminars, professional readings, and membership in appropriate professional organizations.
19. Projects a professional image through appearance and dress.
20. Attendance is regular and adheres to HCESC attendance policy and procedures.
21. Accepts criticism, advice, and recognition; implements activities in response to recommendations or to meet performance improvement goals
22. Perform other duties as assigned.
