Key Responsibilities

Care Coordination - Collaborate with clinical teams to optimize care management.


• Identify specific patient groups that would benefit from enhanced care management.


• Implement pre-visit planning to ensure comprehensive care delivery prior to patient visits.


• Utilize precise, evidence-based reminders to enhance patient care.


• Coordinate care services for selected patients, working closely with families and healthcare providers to achieve optimal outcomes.


• Follow up with patients post-hospitalization to arrange appointments, provide medication education, and clarify after-visit summaries.


• Collaborate with the Care Team to ensure smooth transitions for patients treated in various healthcare settings.


• Engage patients in activities that promote health improvement.


• Educate patients on self-management strategies to empower them in controlling their health.


• Provide health education and counseling to patients and their families.


• Identify obstacles hindering patients from achieving optimal health and collaborate with the Care Team to devise solutions.

Clinical Quality Assurance


• Work closely with healthcare providers to ensure clinical quality remains a priority throughout patient care.


• Participate in quality improvement initiatives and assist in implementing best clinical practices.


• Network with professional colleagues and community organizations to develop effective practices.


• Coordinate with physician leadership to strategize care for high-risk patient populations.


• Monitor resource utilization and collaborate with patients and care teams to promote efficient care.


• Lead the development of office workflows in partnership with physicians to establish care standards.


• Inspire the healthcare team by modeling effective nursing practices and delivering exceptional support.


• Ensure adherence to safe practices and report any concerns.

Qualifications
