Field Community Resource Coordinator

HealthCare Support is actively seeking a Field Community Resource Coordinator
to fill an opening with a Managed Care Organization in Osceola County, FL.

Daily Responsibilities for Field Community Resource Coordinator
  • Perform member and provider outreach and health risk assessment by conducting field visits or initiating outreach calls
  • Serve as a health plan liaison for new/existing community-based organizations
  • Shift: Monday to Friday between 8am to 6pm/ Work Location: Field in Osceola County, FL

Required Qualifications for Field Community Resource Coordinator:
  • High School diploma or GED
  • Active FL Community Health Worker (CHW) Certification
  • Working Knowledge of geographic area, the Social Determinants of Health and known community-based organization/resources
  • Reliable vehicle and clear and active driver's license

Preferred Qualifications for Field Community Resource Coordinator:
  • Bilingual in English and Spanish
  • Motivational Interviewing skills
