Job Summary: Provides in-home services and supportive interventions including, case management, linkage to community resources, and inter-agency coordination of treatment. Conduct assessments and evaluations of clients at intake, during ongoing service, delivery, and termination Formulate relevant goals with behaviorally measurable objectives that serve as indices for evaluating intervention progress and realistic time frames for achievement. Prepare written documentation of assessments, interception plans, progress notes, and discharge summaries in the clients' files with all entries authenticated with dates and times. Plan for, communicate, and document aftercare plans for clients completing program and/or accessing other services in the continuum. Collaborate as a member of an interdisciplinary team, including but not limited to, active participation in program and team meetings, providing support to team colleagues, and engaging in program-related performance improvement processes and activities. Participate in weekly team case conferences and trainings as per Family Preservation Services' requirements. Provide 24/7 coverage to caseload What We Need to See: A Bachelor's degree or a master's degree in social work or other behavioral health discipline from an accredited college or university Two (2) years of experience providing counseling to children and families. A valid driver's license must be insurable by the agency's automobile insurance.
