Housing Case Manager

CarePlus New Jersey is seeking a skilled Housing Case Manager to join our team. As a Housing Case Manager, you will provide care management services for Bergen County residents at risk of homelessness, promoting maintenance of permanent housing, independence, and stability in the community.
Key Responsibilities:- Assess clients at intake, during ongoing service delivery, and at discharge to identify their needs and develop effective care plans.
- Collaborate with service providers to establish comprehensive assessments and care plans that address clients' physical, emotional, and social needs.
- Coordinate continuity of care within the agency, community, jails, hospitals, and other relevant settings to ensure seamless service delivery.
- Formulate comprehensive care plans containing relevant, measurable objectives and realistic time frames for achievement, and perform quarterly reviews of progress and evaluate the effectiveness of goals.
- Provide or link clients to health care services, dental care, benefits, mental health services, substance abuse services, family consultation, crisis intervention, and vocational, educational, or employment services as needed.
- Complete ongoing evaluations/reassessments through regular contact with clients to appraise changing needs and progress towards goals.
- Bachelor's degree with 1 year of experience working with the homeless population.
- 1 year of experience in crisis intervention, counseling, mental health, substance abuse, domestic violence, housing procurement, or job development.
- Valid driver's license and must be insurable under the agency's automobile insurance policy.
