Job Summary

Provide Intensive Case Management to families and individuals that include intake and assessment, specific service deliveries, as applicable, and referral to community organizations and/or component services within the Program for public assistance, health care, employment and supportive services.

Role and Responsibilities
  • Manages, develops and administers policies and procedures for daily operations of the Preferred Communities project.
  • Participates in research and development of the project and manages project activity while making sure guidelines set by funding sources are being met.
  • Develops and implements short and long term planning and activities.
  • Provides direct case management services to clients, including needs assessment, service planning, provision of information and referral services and follow-up with clients.
  • Conducts client intake and assessment; generates service plans; work with clients to ensure short-term goals are met; participates in case conferences.
  • Opens Intensive Case Management case files as referred by resettlement case managers. Receives referrals from Case Managers and Counselors for Healing Circles.
  • Organizes groups of referred clients into Healing Circles and develops activities for each.
  • Coordinates Healing Circle activities with Counselors and Interpreters.
  • Teaches or leads Healing Circle activities.
  • Helps refer and enroll clients in health and mental health services
  • Coordinate service delivery by assisting clients to enroll in services both within the program and with community partners.
  • Maintain a professional appearance and behavior as a representative of the Agency and Program.
  • Discharge duties using independent judgment and discretion.
  • Collects, analyzes, interprets and reports on project data and prepares statistical and narrative reports for services provided.
  • Documents activities with clients in case files.
  • Attend and actively participate in Agency and Program meetings, conferences, in-service trainings and other meetings deemed appropriate.
  • Review USCCB provided webinars, handbooks and other training materials to build case management capacity.
  • Provide interpretation services, both within the Agency/Program and to outside the Agency partners, if applicable.
  • Complete and submit a time accounting document on a daily basis as determined by the director of Refugee Services.
Qualifications and Education Requirements

Education: Minimum: Possession of a four year undergraduate degree (Bachelor Degree) with or without experience.

Experience: Prior Case Management experience in social services field preferred. Experience with people from a culture outside the United States. Empathy for and understanding of refugees preferred.

Registration, licenses, certifications or special training: Fluency in English and a language of populations served by STVCC Refugee Services is desirable. Competency in basic computer programs including all MS Office programs. Prefer ability to work beyond the parameters of posted Agency/Program hours of operation as necessary to complete Job Responsibilities. A valid Michigan Driver License, access to reliable transportation and maintain appropriate insurance.

  • Emotionally able to work with information that may be traumatic in nature, as well as, work with individuals or families in crisis or experiencing trauma.
  • Ability to stand and/or walk up to 8 hours per shift.
  • Work involves talking, stooping, squatting, twisting, climbing, kneeling, hearing, using hands to handle, feel or operate objects, tools, or controls and reach with hands and arms.
  • Vision abilities required by this job include close vision, distance vision and the ability to adjust focus.
  • May be required to push, pull, lift, and/or carry up to 25 pounds.
  • Ability to sit and use computer workstation, including keyboard and visual display terminal for extended periods of time.
  • Noise in the work environment is usually moderately quiet but can be very loud at times.
  • Work in an environment with various degrees of discomfort.
  • Driving as needed throughout the day.
