Services provided may include but are not limited to, bio-psychosocial assessment and evaluation of factors impacting the patient’s ability to participate optimally in the treatment plan, providing education and linkage to additional financial and community resources, advocacy, and counseling services to the patient and short-term counseling to caregivers. The LCSW serves as patient advocate and liaison between other healthcare entities, facilities and community providers, resources and agencies, working collaboratively to ensure seamless transitions across the continuum of care. The LCSW confers and collaborates with physicians in implementing the treatment plan. Responsibilities: • Integrates management and delivery of comprehensive healthcare services. • Performs tasks of a clinical nature within scope of assigned duties and clinical expertise. • Effectively communicates needed information to Providers. • Communicates effectively in both oral and written form, as appropriate for the needs of the audience.
