Key Responsibilities
  • Assess, plan, implement, and evaluate social work care for patients and families to empower them to achieve maximum levels of wellness and independence.
  • Provide holistic social work care in a competent and compassionate manner.
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' social, emotional, and environmental needs.
  • Develop and implement interventions to address patients' social and emotional needs.
  • Provide education and support to patients and families to promote healthy coping mechanisms and self-care strategies.
Requirements
  • Graduate of a Masters Program in Social Work with LCSW or LMSW certification.
  • State of Maine licensure as an LMSW or LCSW.
  • Current certification in Basic Life Support for Healthcare providers.
  • Current, valid driver's license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy.
