Key Responsibilities
  • Provide non-judgmental, person-centered social work services to patients in our HCH sites, including assessment, counseling, and case management.
  • Collaborate with medical and psychiatric providers to develop and implement comprehensive care plans.
  • Work with external community partners and case managers to ensure seamless care coordination.
  • Develop and maintain relationships with patients, families, and community stakeholders to promote dignity and respect.
