COMPETITIVE BENEFTIS PACKAGE INCLUDES:
- Pay range is between $53,100 to $57,800 annually. Can increase based on experience with possible increases after 6 months from date of hire.
- ACRS pays 100% for employee only HMO/Virtual Plus medical benefits. Additional options are covered anywhere from 75% to 85%.

- Employee assistance program
- Sends a welcome packet with brochures and letter to clients referred to the program. Establishes initial communication with referred clients by explaining the benefits of the program and the importance of being actively engaged in their own health and medical needs. Completes opt-in process with program beneficiaries including participation agreements, health-related screenings, and initial home visit assessments as needed.
- Assists potential clients and their families in completing the required enrollment documents. Supports clients and their families through the application process, document submission, and coordination with assigned Care Coordinator. Monitors and tracks due diligence requirements for new program enrollees and current participants. Coordinates with Care Coordinator to complete disenrollment process for beneficiaries, including opt-outs and case closure.
- Collaborates with Care Coordinator/s to provide case-related allowable activities such as health-related screenings and care coordination services to current program participants in-home or at beneficiary's location of choice. Assists Care Coordinator in outreach to engaged beneficiaries to maintain monthly face-to-face meeting requirement.
- Provides supportive services to Health Home or Care Transition program participants under the direction and supervision of the assigned Care Coordinator.
- Maintains thorough and timely documentation of care and activities provided. Ensure all involved parties are updated on completed care and activities to avoid either omission or duplication of services.
- Participates in program's quality improvement, contract compliance, and client service excellence initiatives. Ensures timely communications with program management and care coordinators. Engages in daily case triaging and problem solving.
- Conducts outreach to applicable health care settings and in the community to educate potential clients on Health Home and Care Transition Services. Performs research and organizes community resources to streamline access.
- Assists program manager in outreach and marketing planning and development. Analyze marketing data and make recommendations for future marketing strategies. Participates in creating instructional and informational material regarding program services. Helps in inbound and outbound marketing activities to reach targeted clients through appropriate channels (traditional and digital marketing).
- Attends and participates in all agency, program and team meetings and in-service training.
- Education and Experience:
- Bachelor's degree in social or behavioral science; OR
- Bachelor's degree in relevant field plus two (2) years of providing direct social services; OR
- Associate degree or equivalent in relevant field with four (4) years social services experience. Relevant work experience may substitute for a degree in a relevant field.
- Cultural Humility:
- Strong knowledge of Asian & Native Hawaiian/Pacific Islander (A&NH/PI) cultures required.
- Ability to provide respectful and effective care and services responsive to diverse cultural beliefs and practices.
- Fluency in an A&NH/PI language, Vietnamese preferred.
- Knowledge and Skills:
- Strong interpersonal and customer service skills to engage prospective clients and support their enrollment and care coordination.
- Ability to deliver effective service to employees, teams, collaborators, and clients with diverse cultural beliefs and backgrounds.
- Experience in outreach, community engagement, or healthcare marketing preferred.
- Organizational and problem-solving skills to manage documentation, enrollment processes, and quality improvement activities.
- Technical Skills:
- Proficiency in Microsoft Office applications (Word, Excel, Outlook) and relevant software for data entry and tracking.
- Ability to analyze data and provide recommendations for outreach and marketing strategies.
- Effective internet research and navigation skills to streamline access to community resources.
- Additional Requirements:
- Must have reliable transportation. If using a personal or agency vehicle to fulfill job duties, must have a valid driver's license and be insurable under the ACRS's auto policy. If using a personal vehicle, must have current auto insurance including coverage for work-related business.
WORKING CONDITIONS: The position requires regular mobility within the office or workspace, may involve sitting for extended periods throughout the day and the ability to handle objects, tools, or controls. Occasional lifting or moving of up to 20 pounds may be required. May involve tasks where visual acuity is beneficial. This position is hybrid, with work divided between remote and on-site locations. Approximately 20% of the work will be conducted on-site or in-person (home visits), while the remaining 80% will be performed remotely (telecommuting). These hybrid work expectations are subject to change at the agency's discretion. The noise level in the on-site work environment is moderate.
