Problem Solver and Outreach Worker (San Francisco, CA) ORGANIZATION SUMMARY Catholic Charities is one of the largest, oldest, and most comprehensive nonprofit human services organizations in Northern California, annually reaching more than 60,000 vulnerable individuals of all faiths in San Francisco, San Mateo, and Marin Counties. With a staff of approximately 450, reaching peaks of nearly 600 with seasonal hires, a budget of $54.3 million, and a current governing board of 27, this venerable, fully-accredited, independent 501(c)(3) nonprofit addresses some of the most pressing community needs – homelessness, generational poverty, immigration, inequality, hunger, aging in isolation, and communicable disease -- to name a few – with compassion and services of highest quality. PROGRAM & POSITION SUMMARY Catholic Charities directly supports homeless families in the City and County of San Francisco to effectively engage, advocate, perform service planning, and stabilize vulnerable and at-risk families. The Housing Problem Solver is responsible for: triaging families that enter the Access Points through problem-solving mediation, family reunification or relocation assistance; limited financial assistance; or by assisting families move quickly into housing by conducting housing assessments that assist in prioritizing chronically homeless families. This position also helps by conducting outreach and providing referrals to impact greater housing retention and reduce vulnerability. The Housing Problem Solver will also educate the community and providers about the eligibility and services available to homeless families. Flexible 8 hours shifts between 7:00a/7:00p Monday thru Friday. Working at either Bayview Access Point or Mission Access Point outside of your assigned location is required. ESSENTIAL DUTIES & RESPONSIBILITIES The duties and responsibilities of the Housing Problem Solver position is but not limited to: Community Outreach - help people who are marginalized to tackle the problems that face their local area; specifically, homelessness or being under-housed; Ensuring quality data collection and accurate documentation of multiple reports; Verifying families who self-report their homelessness which could require entering homeless encampments; Participate in related program, agency, and community meetings as assigned; Communicating and working well with all levels of representatives, internally within Catholic Charities and externally with partner organizations; Accompanying individuals or families to appointments as needed; Assessing the overall needs of the family unit to determine realistic goals; helping families cope with the challenges related to unstable housing/homelessness; substance abuse; physical and mental health issues; domestic violence, and sexual and human trafficking; Provide childcare and children’s activities at the Access Point and reception duties as needed; Essential achievements include but are not limited to: Assessments – minimum of seven per week Referrals – minimum of five per week Successful Problem-Solving Resolution – minimum of one per week Trainings – minimum of one per week Community/Street Outreach: Referrals – minimum of five per week Successful Problem-Solving Resolution – minimum of one per week Trainings – minimum of one per week Client Surveys – minimum of five attempts per week Phone Calls – answer, return and complete a minimum of ten per week CARES and ONE System – enter with 100% accuracy all data into computer collection databases Adherence to all Catholic Charities; Access Point, and Department of Homelessness and Supportive Housing (HSH) Policies and Procedures is strongly encouraged. First Aid/CPR Certificate REQUIRED within the first 90 days of employment.
