Summary
The Outreach worker will be responsible for the provision of street and clinic outreach services. This position provides outreach to the members of the community experiencing homelessness to assess clients’ needs and aid clients in accessing resources both at FJV and in the community. Connects individuals with needed health services. Outreach engagement may include short-term crisis intervention, case management, and counseling for clients experiencing homelessness, substance use disorder (SUD), and serious mental illness (SMI) as well as other barriers to housing stabilization. Participates in care management with a multidisciplinary team with the goal of ensuring a client’s ongoing treatment needs are successfully connected with appropriate community-based treatment and housing support programs. The outreach worker will also attend and support case conferencing. The outreach worker will collaborate with the street health team and contribute to street health efforts.
Essential Functions
- Locates and connects homeless individuals/families on the streets to guide them through the coordinated entry system and/or connect with the Street Health Program and Village Health Center services.
- Collects and enters patient information, status, and statistics for reporting purposes in the electronic health record and various reporting databases to improve efficiency of services.
- Performs direct patient outreach in the community by searching the lunch line, walking the neighborhood, checking tents and cars and working with the patient to return to the Health Center for follow-up, as needed, to promote effective coordination of care.
- Drives street health van to support street health team and transport patients.
- Multitask, communication with patients/de-escalation & assessment skills, provide customer service, and trauma-informed care to ensure patient safety and satisfaction.
- Conducts AOD assessments, offers individual counseling, provides referrals and some case management, documents progress notes, and formulates treatment plan goals with clients. Monitors client progress and modifies their treatment plans as needed.
- Participates in Harm Reduction Outreach in the community; delivering Naloxone, safer use supplies, substance use education, and SUD referrals to community members.
- Completes timely clinical documentation and participates in monthly peer review and MAS meetings.
- Offers support through on-call schedule to providers and answers questions about program services.
- Maintain caseload of clients to assure care continuity and improve stability.
- On time completion of assigned training and policies.
- Performs other duties as assigned.
Qualifications
- High School Diploma or GED
- At least 1 year of outreach and/or case management experience working with under-served populations.
- Basic user of MS office
- Participate in annual Tuberculosis screening and/or other screenings when necessary.
- Reasonable accommodations may be granted where appropriate.
- Possesses a valid California driver's license; and has no Class 1 or more than two Class 2 violations within the past 36 months.
- Experience with harm reduction treatment model preferred.
- Registered with an Alcohol and Other Drug certifying body
