Oncology Care Coordinator

The role of the Oncology Care Coordinator is to assist with the coordination of care for oncology patients across the healthcare continuum. Under the direction of an oncology nurse navigator or designated professional, the Oncology Care Coordinato r works collaboratively with the patient, their families and/or caregivers to overcome healthcare barriers and improve access to care.

Duties and Responsibilities (including but not limited to)

  • Review with the patient, their family and/or caregiver services available within the cancer program
  • Identify patient’s logistical needs and assist patients with supportive service referrals, i.e. appointments, community resources, food access, transportation
  • Contact the patient under the direction of the nurse or social worker to review appointments, referrals and additional resources needed
  • Gather medical and financial information, and review patients progress, barriers and concerns with the professional staff
  • Track outcomes and services provided under the direction of the professional staff

Other duties as assigned

Qualifications or Education, Training, and Experience

Education:

Associates Degree of Applied Science in a health related field or medical assistant required

Must successfully complete a recognized patient navigator-training program or provide documentation of the requisite knowledge and skills from previous education and experience to provide patient navigation within one (1) year of hire date.

OPN-CG (Oncology Patient Navigator-Certified Generalist) obtained within 3 years of hire

Experience:

Two years of experience in a healthcare setting required

Experience in working with electronic medical records preferred

Serves the following age groups

  • Adult patient and family members (18-65 years)
  • Geriatric patient and family members (over 65 years)

Knowledge and Skills:

  • Adaptable to new technologies and software.

  • Basic PC skills (MS Word/Outlook/PPT/Excel).

  • Analytical, able to collect and interpret data

  • Knowledge of Federal and State regulations for Medicare and Medicaid and other national and state funded programs.

  • Knowledge of community resources access.

This is a hybrid position with some traveling required. May be working onsite at multiple office locations in WNY

Pay range: $18.00 to $23.00

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within job’s compensation range, and will be determined by considering factors including, but not limited to market data, education, experience, qualifications, and expertise of the individual and internal equity considerations
