About You

The Community Health Worker Manager should have the following qualifications:

Education: Bachelor’s degree or equivalent in human services, public health, social work or similar field with a record of strong academic achievement;

Experience: At least 3 years’ care coordination experience, preferably in greater Buffalo, with at least one year supervisory experience;

Background: Strong, practiced familiarity with Buffalo-area public assistance programs, social service providers, and community-based organizations; Comfort working in a diverse, fast-paced environment; Lived experience in the community / target population strongly preferred

Skills: Experience with electronic medical records, health information exchanges, z-coding, and related systems and tools; Proficiency with Microsoft Office Word and Excel;

Communication: Excellent written and verbal communication skills and experience communicating with and participating in interdisciplinary care teams

About the Job

The Community Health Worker Manager reports to the Vice President of Healthhome with accountability for providing leadership to meet CINQCARE requirements. They should embody CINQCARE’s core values, including, Trusted, Empathetic, Committed, Humble, Creative and Community-Minded.

The CHW Manager will supervise a team of Community Health Workers (CHWs) as they provide non-clinical participant outreach and engagement, screening and assessment, coordination of non-clinical services and supports, family and psychosocial support and health promotion coaching, and monitoring of participant wellbeing.

The Community Health Worker Supervisor will have the following responsibilities:

• Supervise, train and coach a team of CINQCARE Community Health Workers to fulfill non-clinical care coordination and case management with participants

• Process prospective participant referrals and dispatch CHWs to conduct initial screening and enrollment

• Participate in and ensure CHW participation in all required program trainings

• Ensure timely entry of completed HS screening/assessment forms into required data repositories

• Support RN-CM’s completion and update of participant-centered care plans

• Monitor quality of CHW care coordination/case management activities with participants

• Coordinate and monitor care plan implementation and triage changes in status

• Respond to and support participant transitions of care as directed by RN-CM

• Participate in daily huddles and weekly case conferences

• Strengthen lines of referral across other social service partners/providers

The Community Health Worker Manager will also have the following duties:

Leadership: The Community Health Worker Manager will lead in defining and executing strategies and solutions to create business value, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes.

Strategy: The Community Health Worker Manager will establish the business strategy and roadmap to improve outcomes for CINQCARE members.

Collaboration: The Community Health Worker Manager will work closely with others to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQCARE.

Knowledge: The Community Health Worker Manager will provide subject matter expertise in solutions, including determining and recommended approaches, solution deployment and performance evaluation.

Culture: The Community Health Worker Supervisor is accountable for creating a productive, collaborative, safe and inclusive work environment for the team and as part of the larger Company.

The working environment and physical requirements of the job include:

This position requires both in-home and office-based work. The job requires frequent travel for home visits and travel to physician offices, hospitals, sub-acute facilities, community partners and non-home-based market offices in all types of weather conditions. In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.

In this position you will need an ability to travel frequently by car and/or public transportation, the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 30 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
