CHRIS 180 Re-entry program utilizes a targeted case management system with the addition of counseling. The program targets incarcerated moderate- to high-risk offenders ages 25-40 who are tracked to be released from Fulton County Jail and are returning to the community.
This program is designed to address the unique needs of the target population as they return to their families and communities from detention or incarceration. It has been well documented that these young men and women who are incarcerated are often dealing with multiple issues which require a specialized approach. Without the benefit of in-penal and post-release services, these inmates face a difficult uphill battle against a system and environment not supportive of individuals with substance abuse and mental health issues. Not surprisingly, many of these individuals suffer relapses and engage in criminal behavior that lands them back in jail, contributing to the much larger issue of recidivism. Therapy and wraparound combined creates a single point of accountability to serve youth and families in the community through our program while addressing the mental health and substance abuse challenges to ultimately reduce the chances of recidivism.
Role and Responsibilities
The individual who accepts this position must be comfortable working with the population being served and be able to pass a criminal background check and screening for both CHRIS 180 and Fulton County Jail.
The Reentry Specialist assists participants in identifying and gaining access to housing, employment, identification, medical, social, educational, and other services. The Reentry Specialist encourages the use of community resources through referral to appropriate traditional and non-traditional providers, paid, unpaid, and natural supports. Reentry Specialists work in partnership with the individual and their family/caregiver/legal guardian and are responsible for assembling identified team to reach goals set by the individual being served.
The Reentry Specialist must be emotionally stable, able to handle high stress, relate well to a wide variety of individuals, and be able to work as part of a team. The Reentry Specialist must be able to follow, enforce and implement program descriptions as outlined in policy and procedure.
The Reentry Specialist must be able to provide one-to-one interaction with a designated enrollee and their family; act as liaisons and advocates for the individual with other agencies as needed, complete required paperwork, and perform other related duties as required. Reentry Specialists must be flexible in working hours, be able to respond to crisis situations in an effective manner and meet regularly with their supervisor.
This position provides care coordination to consumers/residents/member and the primary duties include but are not limited to the following:
- Demonstrate exceptional engagement skills and wraparound expertise in providing trauma-focused approached treatment to target client population.
- Have an understand of the minimum standards for the population and/or the funding program requirements
- Maintains a caseload of up to 20-25 individuals. All with varying degrees of involvement.
- Engages individual to increase likelihood of follow thru with appointments in the community upon release from jail
- Ensures that individual is met face-to-face within 72 hours, depending on their schedule
- Communicates minimally bi-weekly with supervisor regarding caseload status
- Completes case notes within 48 hours of contact with the consumer.
- Completes a comprehensive intake assessment of the participant to determine services needed, including activities that focus on the need for any medical, educational, social, or other services.
- Develops a plan of care with the client in order to establish goals, needs and strengths in order to reduce recidivism.
- Assessing residents/consumers/members/clients' history of trauma, strengths, and needs.
- Aware of ACE score and implications on service planning
- Complete GAINS assessment as necessary
- Complete all consents and release of information necessary
- Complete satisfaction surveys with client
- Periodic reassessment of the participant to determine ongoing service needs.
- Assembles a team meeting within two (2) weeks/14 days of referral by interviewing the individual, and identifying family members/natural supports/agency representatives and other significant persons as available
- Assess the strengths and needs and promotes strategy development with assistance from team members
- Ensures Plan of Care is submitted via the electronic health record (EHR) within 4 days of the meeting in order for corrections to be completed
- Assists the individual and his/her family in accessing mental health, housing services, employment services, social services, educational services, and other services and supports as identified by the client and team members
- Ensures that the services being provided are responsive to the client vision and needs identified in the Plan of Care and that the family/client is experiencing voice, access and ownership of their plans and services
- Communicate with service providers to overcome barriers within the action plan. If direct communication is not effective, the Reentry Specialist will address ongoing issues with the supervisor to develop a plan to address.
- Monitors the provision and quality of services through the team and modifies the Plan of Care per the guidelines above.
- Seeks community resources first with the assistance of the team and modifies the Plan of Care whenever services or resources need to be added and/or deleted
- Provides or secures support and crisis/emergency services for the child/family. This may be done through face-to-face contact, phone contact or availability by beeper or an on-call system
- Collaborates with other necessary individuals the consumer may have contact with, i.e., Employers, DFCS Case Workers, Probation Officers, Teachers, Judges, Attorneys, Physicians, etc. This means that the Reentry Specialist contacts these key people by telephone frequently, invites them (with adequate notice) to the team meeting, and provides them with copies of completed Plan of Care when this is approved by the client/family
- Ensures that ongoing team meetings are held every 30 days at a minimum and prep work for transition to sustainable service providers of the individuals choosing is assessed at each meeting
- Provide clients with satisfaction surveys for the department to track outcomes every quarter.
- Complete a monthly internal peer audit review
- Complete ongoing review of their assigned consumer's charts and update or input any missing documents as necessary. The consumer's chart is the sole responsibility of the Reentry Specialist and must be aware of the contents in order to assure all required components are included and complete

The Reentry Specialist will have a minimum of a Bachelor's degree in one of the social sciences and will have experience working directly with youth and families with youth having a severe emotional and/or behavioral diagnosis (SED). Preferred experience with adults involved with the penal system.
The Reentry Specialist must have the ability to establish and maintain professional relationships with individuals in the community and professional agencies as well as the ability to interact with various client populations. Reentry Specialists must also have the ability to express ideas effectively and professionally both verbally and in writing. Reentry Specialists must have a dependable vehicle since the position requires travel and insurance coverage beyond minimum liability. Reentry Specialists must exhibit excellent organizational skills to deal with scheduling, budgeting, coordinating services, etc.

Schedule:
This is a full-time position that typically works Monday - Friday from 8:30 am - 5:00 pm, must also be flexible to meet the client's needs.
