What You'll Do

In this role, you will provide robust, family-centered palliative care as a member of the interdisciplinary PATH ("Pain Management, Advocacy, Trust, and Hope") team at Imagine Pediatrics.
  • Patient facing care, which will include
    • Introducing patients and their families to the PATH service
    • Administration of subspecialty palliative care, which will include:
    • Symptom assessment and management, including advanced pharmacologic management when indicated
    • Anticipatory guidance and prognostication for complex disease processes
    • Complex communication surrounding advance care planning
    • End-of-life education and management, when indicated
    • Assessment of child and family coping, and referral to higher level of support where appropriate (e.g. behavioral health, social work, etc).
  • Regular interface with community providers, which will include:
    • Initial outreach to inform about PATH team referral, services offered by PATH team, and gather relevant clinical details
    • Post-visit outreach to disseminate PATH team care plans and anticipated follow-up
  • Hospice communication and management, which will include:
    • Placement of hospice referrals
    • Proactive and reactive engagement with families to describe the hospice benefit
    • Ongoing communication with pediatric hospice providers to ensure patient needs are being met and hospice support is effectively supplemented
    • Maintenance and dissemination of advance care planning documentation (e.g. OOH-DNR, POLST forms, etc)
    • Participation in quality improvement activities as assigned or indicated.
    • Design and implementation of programs which support bereaved caregivers and siblings.
What You Bring & How You Qualify
First and foremost, you're passionate and committed to reimagining pediatric health care and creating a world where every child with complex medical conditions gets the care and support they deserve.
  • Graduate of an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board/national certification.
  • Experience in pediatric palliative care (inpatient or outpatient) is preferred but not required.
  • Demonstrated experience working with life-threatening conditions, chronic disease, hospice and/or end-of-life care, and related situations.
  • Demonstrated experience working with family systems.
  • Ability to think critically about program development, and a willingness to be a leader in development of an innovative care model.
