Overview:

POSITION SUMMARY

Under the direction of the Director, Community Well-Being - Health Partnerships, and as part of the Community Well-Being Team, the Enrollment Specialist is responsible for managing referrals and enrolling participants into the YMCAs Evidence Based and Evidence Informed Health Interventions, including Diabetes Prevention Program, Blood Pressure Self-Monitoring, Kickstart Your Confidence, Livestrong, Nutrition, physical activity programs, and other programs as appropriate. The Enrollment Specialist is also responsible for creating opportunities for expanding referral sources, supporting our billing process, maintaining our database systems, and assisting with administrative processes.

The Enrollment Specialist will deliver professional customer service and relationship building skills to create a welcoming environment in the program. The incumbent will also exhibit the core values of caring, honesty, respect, responsibility, and innovation in all aspects of their work with the YMCA.

Responsibilities:

ESSENTIAL FUNCTIONS

  • Manage referrals: Keep timely record of calls and interactions with referral sources and participants; Answer incoming referrals within 48 hours and make 3 contact attempts within 10 business days; Log information in Referral Management Software and communicate effectively with team. Follow all SOPs for referral management.
  • Manage a portfolio of approximately 1,000 referrals and 250 enrollments annually, including an outreach strategy that includes outbound phone calls, emails or mailings to generate interest in Y programs.
  • Enroll participants in programs, communicating effectively with staff members and participants, following all procedures and policies specific to enrollment.
  • Support accurate billing and administrative efforts for Evidence-Based Health Interventions
  • Manage accurate tracking, outreach, and referral processes for SDoH community needs and referrals.
  • Attend at least 4 community events per year, representing the YMCA with appropriate marketing and enrollment materials
  • Deliver at least one EBHI program regularly. Participate in training and development and follow expectations for delivery of these programs.
  • Comply with all HIPAA protocols for keeping data/information secure.
  • Ensure compliance with grant guidelines.
  • Pull accurate reports from Welld to track referral outcomes, enrollment and other data points necessary for quarterly grant reporting and follow up with supervisor regularly.
  • Follow and enforce all YMCA procedures and policies, including personnel guidelines, safety guidelines, facility access procedures, and membership policies. Carry out emergency plans as necessary.
  • Attend and remain current on all mandatory trainings.
  • Be knowledgeable and supportive of the YMCA annual support campaign

Qualifications:
  • Bachelor's degree preferred.
  • Bilingual preferred. (English and Spanish)
  • History of data management or demonstration of being detail oriented and data/computer savvy, including proficiency in Excel, database management and case management experience preferred.
  • Strong communication and relationship building skills and the ability to build community within a group and have empathy for health seekers
  • Knowledge of basic health, nutrition and fitness is preferred.
  • Committed to and supportive of the Y mission emphasizing support to health seekers.
  • An understanding of behavior change is required.
  • Must be flexible to work with a wide variety of people. Experience in working with the older adult population preferred
  • Familiarity and proficiency with Listen First or motivational interviewing highly encouraged.
