Care Coordinator

Description:

The Care Coordinator will be the first person to meet with the patient.

Proactively identify and work with people, including the elderly and those with long-term cardiopulmonary conditions, to provide coordination and navigation of care and support across health and care services.

Accountable for helping patients keep up with their care plans.

Manage and support patients healthcare needs via phone conversations.

Additional Skills & Qualifications:

Healthcare experience in the medical field is a must have

Must have experience working with patients with pulmonary diseases
