Job Summary:

Hours: 8:30am-5:00pm, Monday -Friday (full time)

Boston Health Care for the Homeless Program (BHCHP) is looking for a talented and compassionate Care Coordinator to provide care coordination services to individuals enrolled in our One Care program-a program for patients with Medicaid and Medicare coverage between the ages of 21-64 years of age. We are also looking for someone who will remain flexible as this role expands.

The Care Coordinator will work on a team with One Care Nurse Care Managers, other Care Coordinators. This role involve collaboration with medical teams to ensure that patients are being linked to needed services to improve their health and overall well-being and are meeting goals outlined on their care plan. The Care Coordinator will also provide administrative support to the team. A significant aspect of this work will be direct patient engagement that will be a combination of telephonic and face to face outreach and coordinating with providers.

Responsibilities:
  • Collaborate with members of One Care team, as well as primary care teams, to support panel of medically-intensive patients with complex health-related social needs
  • Engage One Care patients in services through telephone and face to face outreach; assist with the housing process and stability resources for patients experiencing homelessness, including resource building, referrals to substance use treatment programs, and legal advocacy or housing instability
  • Conduct outreach to shelters and patients' homes to maintain a connection to care
  • Maintain daily communication with the Managed Care Organization (MCO) via web portal and phone, as needed
  • Assist nurse care managers and medical teams with arranging patient appointments for services, accompanying patients to those appointments, if needed, and other related activities; supports patients to meet care needs including assisting patients with housing, income and benefit application.
Qualifications:
  • Bachelor's degree preferred
  • Bilingual in English and Spanish, Haitian Creole or Portuguese speaking skills highly preferred;
  • Experience in case management or human services is preferred; self-directed with the ability to work both independently and as part of a team
  • Computer skills: Proficient with Microsoft Excel Spreadsheets, entering narrative and other data into a database, the use of the Internet to conduct information searches.
  • Strong problem solving and communication skills (written and oral); ability to triage patients' needs and creatively address problems as they arise
Compensation and Benefits:
  • Compensation starting at $22.07 per hour and is commensurate with years of experience.
  • BHCHP full time employees are eligible for our competitive time off policy of 4 weeks' vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional annual compensation of seven thousand five hundred dollars added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
