Description

Summary

Perform all Service Coordination tasks as outlined by the NYS EIP and NYC DOHMH.

Essential Functions

•Regular and reliable attendance is an essential function of this position.

•Guide Families through the initial and ongoing process of the Early Intervention Program

•Provide case management to families with infants and toddlers with developmental delays.

•Ensure families understand their Family Rights, options, and rights to due process.

•Provide a detailed orientation of the Initial and Ongoing process, orient family on the Embedded

Coaching Approach

•Plan and coordinate with families to receive evaluations and services.

•Participate in the development, implementation, and monitoring of the IFSP.

•Monitor the timely and consistent delivery of services, act as liaison between therapists,

families, and providers.

•Develop plans and strategies to meet the needs of families.

•Assess families concerns, priorities and resources; strengths and needs; assist families to access

community resources and services.

•Facilitate and coordinate child's transition to CPSE or other programs, no later than 120 days

prior to age out.

•Follow established EIP timelines, policies, and procedures.

Work Environment

This position operates in a hybrid environment that may include home, ACP-EI office, client homes,

municipality offices.

Physical Demands

•Prolonged periods of sitting at a desk and working on a computer

Travel Required

•Service coordinator's preferred method as needed.

•Mileage reimbursement is not offered.

Education and Experience

A minimum of one of the following educational/service coordination experience credentials:

•Two years' experience in service coordination activities as delineated (voluntary or part-time

experience which can be verified will be accepted) and an additional year of experience in a

service setting with infants and toddlers with developmental delays or disabilities.

•One year of service coordination experience and an associate degree in a Health or Human

Service field

•Bachelor's degree in a health or human service field

Additional Requirements

•Employees in this role are considered essential workers as part of the critical infrastructure.
