The Social Services Coordinator will be responsible for serving as a primary liaison between Centrum Health and The Department of Children and Families Office Economic Self-Sufficiency (ESS) ESS Partner Network. DUTIES & RESPONSIBILITIES * Assist in completing benefit applications for public assistance programs. * Links community residents to appropriate public assistance programs from both public and private funding. * Schedule appointments for follow-up and provides assistance and/or referrals for essential support services such as, but not limited to, housing, financial aid, food stamps, Medicaid, emergency food, WIC, clothing, transportation and day care. * Participate in county, city and other coalitions to promote the available social services programs and identify collaboration opportunities. * Provide community presentations and attend community meetings and outreach events to promote programs. * Process Medicaid renewal for eligible plan members. * Provide both remote and on-site assistance. * Maintains patient confidentiality and complies with HIPAA guidelines. * Assures coverage of Social Services contacts during operating hours. * Register contact in system as applicable. * Accomplish established metrics by the leadership on a daily, weekly and monthly basis. * Manage contacts requesting interpreter services for foreign languages. * Maintain & complete any required training by the company & community program * Interview clients and families. Coordinates and plans programs and activities to meet their social and/or emotional needs. * Comply with government regulations in all areas of responsibility required, I.e. signatures on all forms for Medicaid and LIS. * Follow up with the Department of Children and Families and Social Security Administration on Medicaid and LIS applications promptly. * Prioritize daily duties and adapt quickly to changing responsibilities. * Maintain updated knowledge of existing and new regulations on Medicaid and Social Security. * Assist members as well as prospects in person or on the telephone with questions or concerns. * Maintain an accurate and organized inventory of cases. * Adheres and ensures compliance with Risk Management Program policies and adverse incident reporting. * Understands and interprets policies and procedures. * Ensures the compliance of CMS/AHCA rules and regulations are being observed, as well as the HIPAA regulations as applied to PHI. * Other duties as assigned. EDUCATION AND PROFESSIONAL EXPERIENCE High School Diploma, GED, or equivalent. Associate degree in health, business-related field or comparable experience preferred. Minimum of 2 years experience in healthcare, social services and/or case management. Social services experience, case management, refugee assistance program, public assistance, or limited customer service representative preferred. Good communication, interpersonal and problem-solving skills. Working knowledge of the Florida Medicaid regulations and Social Security Administration to an assigned caseload to obtain and maximize, Qualified Medicare Beneficiary (QMB), Special Low-Income Medicare Beneficiary (SLMB), Qualify Individual (QI1), and Low-Income Subsidy (LIS). Bilingual in English and Spanish.

PROFESSIONAL COMPETENCIES * Computer literate, preferably in the following applications: Word, Excel and Outlook. Ability to respond to a high volume of work and always maintain a polite and professional manner. * Able to function with frequent interruptions. * Ability to prioritize workloads for maximum efficiency. * Customer Service Skills: practicing, valuing and supporting service to both internal and external customers. * Time Management Skills: establishing priorities and accomplishing tasks in a timely manner. * Team Work: ability to work in a team environment to create solutions to problems.

WORK ENVIRONMENT The majority of work responsibilities are performed in an open office setting, carrying out detailed work sitting at a desk/table and working on the computer.
