This position is responsible for assembling information concerning a patient's clinical background and referral needs, assesses quality of referral documents and provides appropriate clinical information to RN Case Managers for admission.Essential job responsibilities include: Coordinates the intake of patient information to ensure timely admissions, treatment, interventions and resource utilization in compliance with policies, procedures and regulatory requirements ensuring timely and effective patient care.Verifies patient payer sources and schedules all hospice and home care admissions in software system and ensures accuracy of information.Responsible for coordination of all patient care; coordinates care and services with other departments/providers from the time of referral to discharge.Assumes added responsibility for special projects, administrative functions as directed and follows through with timely completion of the project.Intake Specialist Minimum Required Qualifications: Education: GED or equivalentRelevant Experience: Two years of related experience in Medicare Home Care/Hospice Current valid driver's license and proof of vehicle insuranceAbility to communicate effectivelyAble to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, Resident's Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and MSDSPreferred Qualifications: Education: Licensed Practical Nurse or Bachelor's Degree in Health Information ManagementEqual Opportunity Employer/Protected Veterans/Individuals with DisabilitiesThe contractor will not discharge or in any other manner discriminate against employees or applicants because they have inquired about, discussed, or disclosed their own pay or the pay of another employee or applicant.
