DescriptionGeneral Description The Intake Specialist establishes records for new persons served. You must be ready to make an incredibly rewarding and strong impact on our community.About the AgencySince 1972, Places for People has helped individuals in the St. Louis community find and define their places in the world.Specifically, our mission is to provide caring, effective services to help those with the greatest challenges recover from mental health illness and associated chronic illnesses.We fundamentally believe that each person we serve deserves a chance to be their best selves and we want to help them get to that point.SOME perks of working at Places for People:Medical, dental, and vision insurance Short and long-term disability insurancePaid Vacation Time (6.67 hrs/month) and Sick Leave (10 hrs/month)Free Employee Assistance Program 8 paid holidaysFlexible work schedule403(b) Retirement Plan contributionMileage reimbursement at the federal rateSupervision available for licensure during work hours and at no costLoan forgiveness through the National Health Service Corps (NHSC) for specific positionsAT&T Wireless membership perksFree on-site fitness center (located at our Soulard campus - available for all staff)Job duties include but are not limited to:Identifies strengths, treatment needs, and any supports (natural or otherwise) of persons servedDetermines needs of persons served by an assessment of areas of daily living and functioning to include: mental health, physical health, substance abuse, employment, housing, and skills in areas such as socialization, problem solving, coping, money management, and time managementCompletes intake documentation within timelines determined by Medicaid and DMH, and any other relevant funding or accrediting bodiesCreates collaborative treatment plan with persons served in an effort to identify goals they wish to achieve while working with PFPInforms persons served of expectations regarding services, focus on services and supports PFP can offer in achieving goalsAttends weekly team meetings to promote and facilitate team coordination and continuity of careAttends weekly supervision to review eligibility decisions and service determinations on new persons servedAssists persons served to reinvigorate a healthy system of emotional and social supports; provide a systemic intervention of helping families and others who may have once provided significant supportCoordinates successful transition of care from Intake to CPR/ACT teams for persons servedCoordinates correspondence with other agencies to obtain and provide client clinical records as neededMaintains quality assurance records and files according to agency certification and accreditation requirementsMonitors clinical documentation of CPR contractors to assure completeness and timeliness of their CPR client recordsCompletes annual training requirements as outlined by agency requirements and supervisorPerforms other related tasks as assigned by the supervisorRequirements Master's Degree in a social services field Working knowledge of the DSM, psychotropic medications, and substance use disorders Experience with Electronic Medical Records and working knowledge of Microsoft Word and Excel.
